1. Visit: http://www.geoaccess.com/guardian/po56/Begin.asp
2. Click on the “Find a Dentist”
3. Select PPO from the “Select a Plan” menu
4. Search by either location or name:

• By location:

• Pick a mileage range
• Enter your zip code, or address, city and state
• Press continue

• By name:

• Enter the dentist’s name
• Pick a mileage range
• Enter your zip code, or address, city and state
• Press continue

5. Select DentalGuard Preferred from the “Choose a Network” menu
6. Enter your preferences:

• Type of Dentist
• Specialty
• Languages
• Gender
• Office Status

7. Press “continue” to view providers meeting your criteria

 

MDG Dentist Search

 

1. Visit: http://www.geoaccess.com/guardian/po56/Begin.asp
2. Click on the “Find a Dentist” icon
3. Select DHMO/Pre-paid from the “Select a Plan” menu
4. Search by either location, name or group name

• By location:

• Pick a mileage range
• Enter your zipcode, or enter your starting address, city and state
• Press continue

• By name

• Enter the dentist’s name
• Pick a mileage range
• Enter your zip code, or enter your starting address, city and state
• Press continue

• Group name

• Enter the group name
• Pick a mileage range
• Enter your zip code, or enter your starting address, city and state
• Press continue

5. Choose your dental network
6. Enter your preferences:

• Type of Dentist
• Specialty
• Languages
• Gender
• Primary care dentist ID

7. Press “continue” to view providers meeting your criteria
