We offer group insurance plans. Forming an insurance group allows us to negotiate better rates from insurance companies. To join the insurance group and enroll in plans, you must be a member and complete an application showing you meet these eligibility requirements.

You are a freelancer, sole proprietor independent contractor, consultant, temporary worker, or are self-employed, or employed part-time.
You will not be considered an independent worker if, at the time of application, you have worked full-time (i.e., 35 hours or more per week) as a W-2 employee for an employer other than a temporary or placement agency, unless you certify that (i) you have not worked in such capacity continuously for more than 18 months prior to the date of the application and (ii) such employment is not scheduled or expected to last more than 18 months from the date of inception.

You must show both that you performed your work and that you were paid for it.
To prove you performed the work, you can submit: timesheets, payroll statements, invoices, employment contracts, letters from clients or employers, or copies of paychecks that show the hours you worked.
To prove you were paid for the work, you can submit: paystubs, payroll statements, copies of paychecks that show the hours you worked, bank statements with the amount highlighted.
If you want health insurance through FIC and are over the age of 65, you will need to show that you are not eligible for Medicare. This is done when you enroll in the plan, not during the eligibility application.
